Doc produces a plastic bag containing Wren's extracted 'baby teeth' — insect mandibles the length of a finger — and Wren runs her tongue along two rough scars on the roof of her mouth where they used to be.
Doc returns first. The narrator remembers his office in pieces: examinations, blood, tests, photos, and a friendliness that never quite lands as safe. The smile does not soothe. The place leaves something behind, maybe the man himself, maybe only the smell, and the memory cannot wash it off. Then the room starts giving itself away. Anatomy books and images crowd in, with locusts, cicadas, hands, and mouths opened up from every angle. A photograph cuts through the blur. The red-haired girl on the exam table looks back angry, and recognition arrives before the narrator can name it. Doc has been waiting. He talks like this is a reunion, says no one needs to be called, and makes secrecy sound almost gentle. The body will not cooperate. Hands fail, eyes drag shut, and the voice slides toward sleep while fear keeps reaching for a phone. Metamorphosis comes in like a chant. The episode shifts from dread into a birth-vision of wombs, seeds, cocoons, soil, and creatures hitting the dark. Light is no rescue. It floods in as something strange, pushing hands, tendrils, leaves, wings, breath, and a scream into one awful arrival. Then the dream lets go. The narrator wakes to a bare bulb, a cold room, missing windows, and medical restraints. The table says enough. Bound arms, fixed feet, a strap across the chest, and a sheet over exposed skin make the room feel clinical, ritualized, and hungry.
