James Sinclair's Business Broadcast · James Sinclair

£40k/Month Foot Clinic with Massive National Potential (4k)

November 2, 2025·1 hr 1 min·5 clips
James Sinclair proposes a McDonald's-style franchise model to scale Foot Medic nationally, flipping Damien's entire business strategy.
The setup is odd. A podiatry operator has five clinics, two owner-occupier sites, cash flow pressure, and a plan for a national chain. Then comes the bit that makes the host sit up. The owner is limiting revenue per shop because he worries some patients cannot afford the service, and that a future buyer might not afford the business either. The host does not let that drift past. He calls it madness and pulls in Specsavers as the comparison point. The goal becomes cleaner: build a repeatable foot-health model, not a collection of clinics held back by nervous pricing. Demand is the useful surprise. Asked whether a new town can fill quickly, the guest says yes, because NHS podiatry is mainly serving high-risk patients now. That changes the mood. Everyday problems like nails, hard skin, biomechanical issues, and gout leave plenty for private clinics to handle. JB gets teased, the joke lands, and then the business point comes straight back. The market is broad. Demand is not the scary part. The numbers are. A shop doing £360,000 in revenue opens up questions about VAT exposure, labor percentage, and what the clinic-level model should really look like. VAT gets a grim aside; the host doubts healthcare will become VATable because the patient consequences would be ugly. The bigger problem is ownership. The guest's partner route sounds flimsy if partners lack the money and vision to buy a clinic. By the end, the cash flow worry has turned into a sharper challenge: stop guarding the ceiling, prove the economics, and build something people can actually scale.

As heard by us

A foot-clinic scale question that turns cash flow into a franchise conversation.

The discussion centers on a five-shop podiatry business and the harder question behind the growth story: whether the founder is ready to scale or still operating like a cautious local owner.
