The episode opens with Creepy podcast host John introducing a feed drop of How I Died, a series about a coroner with a secret ability to speak to the dead in the fictional town of Springfield, which has an unusually high mortality rate. Dr. John Spacer arrives at Springfield's hospital pathology lab for his first shift and is let in by an unnamed staffer before meeting Sheriff Fran Crowley, who delivers the body of a woman found unresponsive after a 911 call from her home. The victim's husband's car is missing and the couple's daughter is unaccounted for, leading the sheriff to suspect the husband poisoned his wife and fled with the child. Once alone with the body, Spacer encounters the ghost of Maggie Lane, who is confused and frightened to discover she is dead. Maggie immediately blames her husband Mark, describing him as an abusive alcoholic who made their lives miserable. She claims the family sat down to dinner, she had an allergic reaction, and she watched her daughter Danny's face fill with fear as Maggie collapsed. Spacer's examination confirms death by anaphylaxis. He notices an indent on Maggie's wrist consistent with a medical alert bracelet and finds large amounts of codeine along with traces of antidepressants and antipsychotic medications in her system. Maggie insists the whole family was deathly allergic to shellfish and that Mark, Danny, and she all ate the same food, meaning all three should have died. Sheriff Crowley returns with news that Danny, age eleven, drove her unconscious father to the ER. Mark is responding to treatment but Danny has collapsed and is not responding to steroids. Spacer deduces that Danny may have been given codeine to numb the pain of the allergic reaction and calls the ER to recommend a stomach pump, potentially saving the girl's life. The critical evidence emerges when Spacer examines Maggie's purse, found in her car rather than in the house. Inside are three EpiPens prescribed to each family member and a bottle of codeine prescribed to Maggie. The EpiPens were deliberately left in the car, away from the dinner table. Combined with the removed medical alert bracelet and the codeine in Maggie's system, the evidence suggests Maggie herself orchestrated the family's exposure to shellfish as a murder-suicide, motivated by a desire to spare her daughter from a life she saw as an inescapable cycle of abuse. Sheriff Crowley warns Spacer that Springfield has the nation's highest rate of unsolved deaths and that no coroner lasts long in the position. The episode ends with Spacer preparing to harvest Maggie's organs as a registered donor, with the possibility of saving Danny's life if a transplant is needed. Maggie's ghost protests as the episode closes.
